門票登記須知 Notice to Applicant

1. 此表格用作登記預留本節目門票。門票數量有限，每人限取 4張。先到先得，額滿即止。每個電郵地址只限登記一次。This form serves for the registration of admission ticket of the captioned programme. Free tickets are distributed on a first-come-first-served basis, each patron may get a maximum of 4 tickets. Each eligible email account can only be registered once.

2. 成功登記人士將會收到確認電郵。如有疑問，請於辦公時間內致電3145 1636 或電郵 pcscpo1@lcsd.gov.hk 查詢。Confirmation email will be issued upon successful registration. For enquiries, please contact the Office at 3145 1636 during office hours or email to pcscpo1@lcsd.gov.hk.

3. 主辦單位保留拍攝及錄影活動情況的權利，有關相片及錄影片將用作日後宜傳、製作節目及紀錄之用。 The Presenter reserves the right to capture and record the event. The photos and videos taken will be used for future publicity, programme production and documentation purposes.

4. 如遇特殊情況，主辨單位保留更改節目內容、日期、地點及演出者的最終決定權。Programmes are subject to change without prior notice.

5. 節目內容及資料不反映康樂及文化事務署的意見。The content does not represent the views of the Leisure and Cultural Services Department.
